Rotator cuff tears are common shoulder injuries that can cause pain, weakness, limited range of motion, and difficulty with everyday activities like lifting, reaching, or sleeping comfortably. The rotator cuff is made up of muscles and tendons that help stabilize the shoulder and support smooth arm movement.

Some rotator cuff tears can be managed without surgery, but more severe cases may require surgical repair. Even after surgery, healing can be challenging because tendons have a limited blood supply, which may slow the repair process. Why PRP Is Being Studied

Platelet-rich plasma, or PRP, is a blood-derived treatment that contains a concentrated amount of platelets. Platelets are known for helping blood clot, but they also release growth factors and signaling proteins that may support tissue repair.

In orthopedic and sports medicine research, PRP is being studied for its potential to influence inflammation, collagen production, blood vessel formation, and cell activity. For rotator cuff surgery, researchers are especially interested in whether PRP may help support tendon-to-bone healing after repair.

How the Study Was Designed

This 2025 systematic review and meta-analysis evaluated PRP use in patients undergoing rotator cuff surgery. The researchers searched four major databases and identified 2,709 studies. After screening, 22 studies were included in the review, and 13 were included in the meta-analysis.

The study focused on randomized controlled trials comparing rotator cuff repair with PRP to rotator cuff repair without PRP. Researchers evaluated pain, shoulder function, patient-reported outcomes, and retear rates over time.

What Researchers Found

Overall, the review found that PRP showed encouraging results when used alongside rotator cuff surgery. PRP was associated with significantly reduced pain scores at 6 and 12 months after surgery. Functional improvements were also seen using the UCLA Shoulder Rating Scale at 3, 6, and 12 months.

One of the most notable findings involved tendon retears. The study found that PRP significantly reduced retear rates at 24 months, with a mean risk reduction of 15.03%. This suggests that PRP may help support longer-term tendon healing after rotator cuff repair.

Not every outcome improved consistently across all time points, and some measures showed stronger results than others. However, the overall findings suggest that PRP may provide meaningful support during recovery, especially for pain reduction and tendon healing.

How PRP May Support Tendon Repair

PRP may support healing because it contains platelets, fibrin, clotting factors, and growth factors that help guide tissue repair. After rotator cuff surgery, the body must rebuild tissue, regulate inflammation, form new blood vessels, and strengthen the repaired tendon over time.

PRP may help support this process by encouraging cell activity, collagen production, and inflammation regulation. Collagen is especially important because it gives tendons structure and strength. PRP may also support angiogenesis, or the formation of new blood vessels. Improved blood supply may help deliver oxygen, nutrients, and repair signals to the healing tissue.

Rather than working through one single pathway, PRP may support several natural healing processes at the same time.

Why These Findings Matter

This study is important because rotator cuff repair is common, and tendon healing remains one of the biggest challenges after surgery. While surgical technique and physical therapy are essential, biologic support may offer another way to improve recovery outcomes.

The reduction in retear rates is especially meaningful because a retear can lead to continued pain, weakness, limited shoulder function, and possible need for additional treatment.

For regenerative orthopedic medicine, this research supports the growing interest in PRP as an adjunct therapy, meaning it may help support standard surgical repair rather than replace it.

What Still Needs to Be Studied

Although the results are promising, the authors noted that PRP preparation and application methods varied across the included studies. Differences in platelet concentration, leukocyte content, growth factor levels, timing, and surgical technique may all affect outcomes.

Future research should focus on standardizing PRP protocols, identifying which patients may benefit most, and determining the best timing and dosing approach. Longer-term studies may also help clarify how durable PRP’s effects are after rotator cuff repair.

This systematic review and meta-analysis suggests that platelet-rich plasma may provide helpful support when used with rotator cuff surgery. PRP was associated with reduced pain at certain follow-up points, improved shoulder function in some measures, and lower retear rates at 24 months.
